Found 1988 $2 coin with a large 4 stamped on the neck of queen. can anyone tell me more of this, or has it been tampered with?

Post-mint damage, not a mint error.

It's just somebody with some metal punches, probably using a coin to test them with before they use them on the "real thing" (whatever that would have been).
