To be referred to as a 'variety' does the repeated variation have to occur on the Master Hub? Or, can it also occur on the Working Hub? Also I would like to know how many of a variation is needed to be found to be attributable? Would a die gouge like the one in the attached photo, that is found on multiple coins be considered a variety?

It could be a die dent or a die gouge. Not a variety. Just a die event. A variety will be on the first coin struck with the die as in a Doubled Die, RPM, Over date, Over mintmark. The first strike has these die has the variety on them. A die event occurs later in the life of the die. (no premium for this)
