1998-D Nickel What Cause This Damage To The Mint Mark?

I posted this 1998d nickel earlier, was wondering about the mint mark. Coop said, no way it could be a "RPM", and I totally agree.The nines, and the eight,are reduced, so "MD"?. The mint mark shows the image of three "Ds", so what cause this to happen?, if it's not an "RPM", then what is it?, any thoughts? "THANKS"..
