I can see a dropped letter fall from the die and onto the flat surface of a planchet before striking but to fall on the upturned edge of the planchet and balance there perfectly before the die strike, Or would it just have to be leaning on the upset edge just right? Just throwing it out there for consideration....
Got this from the CCF search:
"A "dropped filling" error occurs when a plug of compacted die fill falls out of a recess in the die face (usually a number or a letter) and lands on a planchet. Usually the plug falls out of the coin after the strike"

I don't see any obvious damage from the result of a counter stamp on the opposite side.
