To me it is an obvious case of the operator, noticing grease on the central areas of the die and by wiping it off the deeper areas, it move the grease to the side of the die, filling parts of the outer devices. Thus the weakness on both sides of the die. If he had realized this, he probably would had removed and cleaned the dies, but didn't notice it or didn't care. On with the run.

Thanks for the replies. I didn't know very much about " Struck Through Grease" errors. Appreciate the link and the info.
