Yes, I suspect it was a coin - depending on the size, maybe a sestertius of Faustina II or one of the other ladies of the Roman Empire, with a seated deity - maybe Salus? - reverse. The Roman experts will, hopefully, chime in with thoughts.
"Was" a coin. I suspect it was worn almost completely down through the years and, at some point in the relatively recent past, the remaining imagery was enhanced through some brutal tooling.
