That's a pre-die break depression. The die is bulged outward and slightly cracked, a few strikes later that piece of the die would have fallen out.

Looks like Tom stuck his foot out the window. That's a common place for die chips, so I'd say it's a die chip about to chip away, basically what Tanman wrote.

Could be a clash mark that was missed when they were polishing the die or a new chip starting?

That's what I thought at first Coop but I am no expert. I haven't come across any like this just yet.

It would have to be s mild rotation to be a clash, but the obvious answer there is not other evidence of this happening. But seeing if confirms it is not the issue as mentioned.
