Not an error .
Damaged coin.
"Dipping" is not considered cleaning...

Agree, post strike damage. The coin didn't leave the striking chamber in that condition.

The rim on the reverse has been ground down.
You can't have a rim on only one side without intentional damage.

That coin got scrubbed by something on the reverse.
Looks intentional, like somebody took a grinder and used it around the perimeter of it.
