1995 D Quarter Possible Error S - Coin Community Forum

It's got a lot going on with it and I was wondering if someone could better explain what all is going on. It's definitely off centered, the mint mark is barley visible. Does it looks like a little bird? I have same year of coin except it's a 1995 P and it really looks like a bird .
