1995-D-Quarter-Possible-Error-S

I came across this 1995 D Quarter. It's got a lot going on with it and I was wondering if someone could better explain what all is going on. It's definitely off centered, the mint mark is barley visible. The I in Liberty is or looks split in half.

Not any errors, just damage and circulation wear.
It is not off centre enough to make any difference.

This got my attention when I look at the back of the coin underneath the right wings there's a mark . Does it looks like a little bird? I have same year of coin except it's a 1995 P and it really looks like a bird . It's probably I tip all but I will post it soon as soon as I get a good photo without taking a shot through the microscope.
