Its hare to tall from the photos on the ear. Can you get a closer picture of the ear?

The hand doesn't looked doubled from the picture to me.

The scratch on the cheek looks like PMD to me.

Depends. If it is flat, then it is probably Machine Doubling. But if it is raised, it might be a DDO:
2017-Quarter-Doubling-Behind-His-Ear?
On these examples of DDOs, with the affected areas raised above the fields or above the earlobes.
