I can't find much on this DDO and DDR 1971 S Proof Quarter. The only listing is for 1971-S 25¢ Pr WDDR-001 https://doubleddie.com/2316437.html

This is in the same package as what I believe to be a DDO 1971 S Proof Penny. 1971-S 1¢ Pr WDDO 004 (in previous post).

That looks like MD to me, notice how the apparent 'doubling' is heading towards the rim and the thinning of the devices

I agree, Machine Doubling and lots of glare. Try a defused lighting source,

I think what you are seeing is glare from your lighting source.
