I've never understood how a D/S or an S/D mint mark overstrike can happen. Can someone please enlighten me?

All the mint marks were added to the working dies by hand in Philadelphia. The mint worker grabbed the wrong punch, then fixed it.

Or in the case of the 38 D/S and 1955 D/S nickels, there were probably some unhardened S mint dies at Philadelphia at the end of the 37 & 54. Since San Francisco wasn't going to be striking Buffalo nickels in 1938 and any nickels in 1955, rather than let the dies go to waste they punched D's into them and shipped them to Denver.
