So, walk me through this. Coin is broad struck due to an unseated collar, or no collar. Then the coin is clipped because of the (out of sequence) action of the collar?

The coin is indeed struck without the collar being deployed. The flat edge is not a clip. It is where the expanding perimeter of the coin contacted the protruding portion of an off-center coin pinned by the adjacent die pair.
