Other than sometimes assigning questionable grades, I know the various TPG's occasionally get the basic info on the coin just plain wrong - I've seen TPG holders with incorrect attributes, date, mint mark, etc. I've even seen a few with the wrong denomination or (amazingly) the wrong coin altogether.

Years ago I saw two Infantry Soldier modern commemorative dollars labeled as U.S. Army commemoratives. They were on sale on ebay at the same time by two different sellers. I messaged both sellers, and while one removed it and said they were sending it to be re-holdered, the other listed it again at a higher price as a rare holder error .

The following coin just showed up in a dealer's new inventory list, and they did flag it as having the incorrect date on the holder. I'm including the link from the NGC site, where you can see the 1881 Liberty half-eagle labeled as an 1897. Seems like quality control was a little lacking that day...

The TPGs call them "mechanical errors." I had a 1782 dated coin they labeled 1784 by mistake. Called them up and they took care of it for me. It happens.
