1998 D LMC with missing material at the edge. If you couldn't see the line on the rim, or if you seen plier teeth I'd assume it was some dummy with nothing better to do. It looks like there wasn't enough material at time of minting.

It is damage. Note how it extends out beyond the normal outside rim of the coin. It was normal, now altered with damage to the coin. Just a face value coin now.

yep, damage to the coin. it got squished somehow. you can see how the metal swelled out beyond the normal circle of the coin.
