This coin has a great design--John the Baptist in such an important scene! Do you see this as a "straight grading" one, or would the marks behind the head on the obverse "details" it?

How strictly are these papal states coins graded?

To me it looks like the coin would grade AU details, mount removed.
But what a cool coin

As @Keith says, in addition to the blemish behind the head (not clear to me if this is PMD or a planchet defect), there looks like damage from mount removal at 3:00.
I would buy this coin in a heartbeat if I were interested in the reverse scene and the price were right. It is a somewhat atypical depiction of John.
There is also a piastre of the Grand Duchy of Tuscany struck around the same time, with a scene of John baptizing Jesus:
