So, I found this a while ago, and hung onto it wondering what happened to it? I still haven't figured it out, but thought maybe a lamination or planchet error or do I have another damaged coin on my hands?

There is quite a bit of flakes on reverse side like it's chipping away. There is what looks like pieces of the outside rim melted around the inside facing. Honestly, really not sure what I am looking at.... can anyone help?

Unlike those click bait you tube videos, finding errors isn't easy. Even minor ones that rarely add any value like small die cracks and chips aren't common.

Try to learn as much as you can about the die making and minting process. Read the posts and pay attention to links about minting and die making. Bookmark the good ones and refer to them often. This will help you from wasting your time on damaged coins
