You are looking at incuse marks on those areas. They are contact marks from another coin. On a doubled device it will have some of the hubs design on the affected area, and be raised.
On the 1997 Philly DDO, that the affected area is raised above the earlobe:
That is how you can see a doubled ear. It will be raised above the fields or above the earlobe/design area.
