It looks like it has shot by a tiny rifle. entering reverse exiting obverse.
some kind of punch tool maybe. in an odd spot to be done on purpose though. only one hurt was the E on reverse. george and bird were near misses.

Hunting on Quarters, LOL. An Criminal do this. This was, without hole an 1992 transition design. Now it is a hole Quarter LOL.
