1994 D Quarter Clad Issue?

Found this recently. I'm thinking it may be missing. Clad error but it doesn't look like the others that I see that are missing it. It also has these large dimples on one side. Only one or two seem to be affecting the opposite side where you can see a bump or two. Not really sure what to make of it. I also weighed it and it is 5.56 grams which I believe is within the allotted amount. Is it just PMD? Any help is appreciated.

I forgot to add there is no reading on the sides either. Here is a few pics. It also doesn't show the other layer.

When you apply heat to something it bubbles, that is what happened to your coin.

Looks like you got a twofer. Heat caused the bulges and someone filed the edge.
