JC, you have led this old horse to water so many times, I finally took a big drink. I guess I never thought of it this way. It is an error because it wasn't supposed to be this way, and it wasn't anything man made. The die starts to get damaged, then progress's until the die is either replaced or breaks.The die didn't look that (i.e. having a chip or Cud) before it started striking the very first coin.
