The history is simple, and without to go in many technical detais I will give by years, Mint and denominations.

1986 start the experiments
1996 Denver use for cents to quarters ( they has theirs own facillity to produce Hubs and Dies)
1998 Phill start also for cents to quarters
1999 The Halves was added to this procedure

Curiosity: The last double squeeze was for the SBA dollar in 1999. Those Dollards has some verynice last DD's as start in 1909 with the new hubbing and the coins modern era.
