There is no way this could have occurred at the mint - so however it came to be its a damaged coin.

It was smashed against another nickel, hence the reversed ELLO, and it looks like steps and building parts too. Whatever was doing the smashing caused the damage to this coin's reverse, and it appears that it got bent too. Maybe the other coin turned out better after this garage job.
