I'm not certain I understand the question, but most if not all back-to-front offset errors show the serial number.

Agree, almost always the S/N is clearly visible.

No, that is of course if it's not a third print offset error which is extremely rare. The overwhelming majority of offset errors are generated by the first (back) or second (face) print runs.
