Probably a die chip. For a moment, it almost looked like a folded over lamination. But it looks pretty solid, so die chip seems more reasonable.

Beneath the "R" is part of the design, should be there like that.

I think it IS part of the design. I just found another pretty much like it. ...no error.

Tacc is right. The sleeve or some other cloth comes underneath the letter. It's one of them thar fancy-fied new designs!
