This coin is very strange. The planchette is for a nickel, not cladded and no rim ridges, just a rolled up flat edge that is pushed up and down around the full rim the same thickness. The diameter is that of a nickel, but the obverse is Jefferson and the reverse is the normal quarter eagle of 1964. To me this must be extremely rare in that not only was the planchet punched by a 1964 D Quarter front and back, but apparently squashed out the edge to the size of a quarter, then this planchette somehow wound up back in the edge rolling for the nickel. I cannot imagine the odds of that happening and wonder if anyone here has another explanation?

to the CCF Could you take your coin out of the 2x2 and give us a cropped photo of each side. Also the weight to two decimal points is required. Looks like it may be a Dryer Coin or spooned but would like more information.

to the CCF It's definitely not a nickel planchet error. Agree with JimmyD requests for info. Then we can help.
Take a look at my other hobby ... http://www.jk-dk.art

To CCF! It's either a Dryer Coin or it was spooned. Either way, it's not a mint error. It's only worth silver melt value. It's still a keeper, since it's 90% silver.
Errers and Varietys.

The rim looks more like "spooned" / "hammered" but still just PMD.

I agree that it's more likely to be spooned than a Dryer Coin. A 90% silver quarter is a popular coin to form into a silver ring.
