What you're seeing is Machine Doubling. Proof coins are struck more than once, so the chances of Machine Doubling to happen is more likely.

Proofs ARE supposed to be perfect, but the QC isn't as stringent as it was before the '80's...when each proof coin was hand inspected before leaving the mint.

Proofs are stuck twice, so doubling seems to be more common than expected. I do not believe it fails QC or else they would have a very high fallout rate.

Well, they are supposed to be of superior quality compared to business strike because as it's been mentioned they are struck twice. And the Machine Doubling has been one of the most common imperfection on Proofs.

They are stated to be of high quality but all coins that are struck can have errors or have varieties. It's just that they are farther and fewer in between compared to the Billions of business strikes.

Here's a proof nickel from 1971 when QC was more stringent...so mistakes still get out of the mint.
