As you can probably tell by the pirate-shaped holder the coin has been embedded in, it's a "pirate coin". Specifically, a Spanish-colonial gold doubloon or 2 escudos coin, from sometime around the 1600s or 1700s.

Unfortunately, there's no way to tell whether it's genuine or not from those pics; it's quite likely that it's actually a replica doubloon made specifically for use in jewellery items. We'd need to see a clearer pic of the back side before having a guess at exactly where and when the coin was made and whether or not it's genuine.
