OK Just in from the ANA show in Texas
PCGS will now scan all coins sent in for grading noting all PMD and have stated that even if you crack the case and send it back to them they will be able to ID the coin and the grade that it was given.

This is said to avoid reviews and to improve on some of the inconsistency in grading. As well as to compete with CAC

Not all coins. Just some, the choice made by the submitter. An example of a coin in a Secure Plus holder:
