Not sure if this one is an error or not, I can't say I have ever seen one like this.... It doesn't look like it happened after it was minted to me.. Any ideas?

The weight will confirm it's likely post mint damage... looks like it was grated in the Parmesan grater.

It is impossible for it to have been produced in the mint like this.

A coin is made by squeezing a blank piece of metal between two dies. If one of the dies is "missing", then there's nothing for the other die to squeeze against, and no coin can be made.
