does that mean it is from italy ?
Short answer: yes.
Sicily, the large island in the Mediterranean Sea now owned by Italy, was ruled by the kings of Spain up to 1713. "PHILIPPVS V" is king Philip V of Spain. Though ruled by Spain, Sicily is still classified as an "Italian State" and that's where it's filed in the Krause (KM) catalogue.
Your coin, however, is not listed in Krause, because they haven't got around to listing pre-1750 minors from most of the Italian States yet.
"Grano" is the denomination. There were 6 cavalli to the grano, 20 grani to the tari and 12 tari to the scudo, which was equivalent to a Spanish dollar.

worth keeping ? slabbing?
Keeping, yes; it's certainly not rubbish. Grani from the 1730s are listed at $10 in VG conditon; the price for this one probably isn't too far away from that.
Slabbing, probably not worth it.
