I fell like a know this dealer really well, and he has been honest in the past, but should I be a little concerned if I asked how much his proof 2009 territories (silver, 6 quarters) set was, and he said $45?

Same guy also didn't reveal a group of shiny Peace dollars was milled until I noted it. No indicator on the signage or anything.

Opinions? Just innocent errors? I never haggle with this guy. Should I?

Yeah,if you show that you know your stuff, he'll be more likely to give you better prices or to point out stuff that some people don't notice or care about.

It seems to me like you are becoming a collector that is very knowledgeable. That comes with study and comparison of coins. Some people never get a trained eye but keep trying to improve. When at a coin show or coin shop I feel that you should be able to haggle, with respect, if you want the coin. Predetermine what you want to pay and make a fair offer. A ridiculous offer will usually shut the door. Its a beautiful happening when buyer and seller agree on a price. Good Luck!

You may point out that up until very recently, the mint had the 2009 silvers selling for $29.99. They are sold out now but that is a significant price increase for something that was just selling for $15 cheaper a week ago. I wouldn't do it rudely or demand a cheaper price, if he wants the sell he will meet somewhere in the middle.
