I have an unusual Vespasian Dupondius COS IIII (72 AD). As far as I can tell there is no alter and Pax appears to be pointing.

Your coin would appear to match an entry in the new RIC II, RIC II (New) 393, Pax standing left holding branch and caduceus. It is rated as R2, which means that it is rare and cites specimens from Vienna and Baranowsky list 3, 1996 220.
