It happens often enough..sometimes you can straighten them out with vibration such as an electric toothbrush.

The confusing part is that the rotated obverse and reverse aren't noted on the slab.

why would it be noted? it's not impacting the coin, and it may have happened after the coin was slabbed

What is the matter with all of you. Don't you realize that this is done purposely for those people laying down while looking at those types of slabs? Also, for the people that may have installed their computer monitor sideways.
Now just lay down and you'll see all looks normal. Or turn your monitor sideways. SEE?
