Here is another error from Malaysia that must have been a assisted error from their mint (more than likely on the third shift). This is a 2007 flip-over quad-strike, all strikes off center on a edge lettered planchet...
At $50.00 I hope I didn't over pay as I was awe struck when bidding on this piece...

This is more likely than not one of the many custom made error coins that have made their way onto the market in the past few years. Mostly see them on ebay though.
