It is flat and not rounded. So I would say machine doubled damaged.

Why only on the mint mark and no where else?

Probably the best explanation is that the mint mark was punched into the die. The depth was deeper than the other deep devices. So during ejection the dies must have tapped the mint mark because it is higher than the other devices.
