Is this a very extreme example of a lamination error or something else? Also, I noticed when I was placing it in a 2X2 that it was slightly smaller than a regular nickel, does this mean anything? (see last picture)

Have you compared the diameter of this nickel to a normal one?
I'm guessing this is post-mint damage, where the coin was tumbled in a commercial dryer for a long time.
This has been discussed before where the rims get hammered in so much that a layer of metal moves in towards the center of the coin.

Hi,

Bingo! It's a " Dryer Coin" and is damaged.

Here is a picture of a cent that came out of the fin in a commercial clothes dryer. It was tumbling for months with other coins.

Thanks, Bill

Fair enough, thanks for the help. The only reason I though it might be a lamination was due to the ring on the reverse that looked like the center had been torn out. I guess this is just a side effect of the dryer.
