I have a 1904 Barber half dollar with a double strike S/O.

Can anyone give me any information on this?

It is hard to get a picture of it, I keep getting a glare from the plastic case. I scanned it but it is hard to see the double strike.

How can a grading service overlook such a coin error? Kinda hard to miss, isn't it?
