I have several nickel dollars with the chrome die plating flaked off (like a mortar set), the edges are usually ragged, and there is never this much relief with that type of error. Look in the 67th Charlton, in the nickel dollar variety section, for 1986.
So far, the best I can think of is a piece of scrap nickel fell on the die, and into the nose area of the die (remember the devices are incuse on the die). Then, a regular planchet enters the striking chamber and the coin is struck with extra metal attached. If it was a die chip or something, then I should have more than one in a mint box...
If that is not what happened, then the mystery continues...
