This D looks like it has a large shift to the southwest. Is it possible someone made a mistake and repunched a normal 42-D. Is what we are seeing a smashed normal D?
I feel it is Machine Doubling also. Strong push doubling northeast. Other devices on the reverse will probably show the same thing? They are not present though. If it were a RPM, it would be raised above the field. This is not, but flattened to the field area.
