Yes. Its all damage. Just a coincidence it resemble a mouth. Plus if it was you that cleaned that field and scratched the coin, you dont want to do that. If it was something valuable, it would drop the price.

I haven't done anything to it at all. Also, not sure if it makes a difference but you cannot see the 3x with the blind eye. Only under a magnifier and the picture taken with an iPhone.

A nicked/dinged Lincoln Cent. Copper is a relatively soft, reactive metal--so, surface hits and exposure to the environment show up nicely. Post-mint damage in my opinion. Forty-eight years in circulation--the coin has been through a lot. At least it avoided the prolonged parking lot treatment.

A close-up, high-resolution photo of the 'phantom O/D/5/S' area might show something more.

Deliberately cleaned? I'm thinking not. I've searched well over a million Lincoln cents and your cent looks like it falls into the range of average, uncleaned early '70s Lincolns.
