Moonlight mint has one of these presses the mint uses, they bought it surplus from Denver Mint I think. I'm not claiming they did it, just saying they have the capability to do it, and it would look like a U.S. mint "error" (is something intentional even an error?)
Anyways, I don't see why this couldn't be one of their "fantasy pieces" really, messing around with the press and having some beers.
All I know is something like this doesn't happen by "error".

Here's the sanding disc. 1986 silver eagle   Looking on sold records there are coins to match a few of these sanding discs. There must have been a few of them. Some obverse and some reverse.

And the double denomination is broad struck out of the collar for the Lincoln Cent.
