I can't see the FG initials on the reverse. Is this an error?

Most likely the result of a grease-filled die or over-zealous die polishing. Not an error.

Or the third possibility, die wear or a combination of all three. Anything is possible then.

No error - no added value. Grease-filed dies happen all the time and can remove many things.

For a Grease Filled Die to have any extra value it has to be fairly extreme. Same goes for most struck through coins. I have come across a few Canadian struck through but americain coins missing FG seem fairly common, as well I have noticed at 12oclock on most americain coinage you see very weak strikes and grease filling if th die.
