It looks like it was jammed in some sort of mechanical device like a vending machine.

The second strike looks like it was done with a hammer and an anvil altering the coin. It was not a mint error. But intentional damage to the coin, trying to make it look like a mint error. When they realized, it wasn't fooling anyone, it was spent.
