I have probably searched through hundreds of 1965 silver dollars.
Never seen anything like this - too bad its in poor condition.

What caused this to happen?
Has anybody seen anything like this before?

My thought:
-combination of slight die variation, and impact damage affecting the top of the '6'.

It's just that the dings make the 6 appear to be canted, when it's just missing sections of it.
