What makes the A's different on the reverse in USA? Nothing on the reverse indicates that the lettering would be PMD. I'm not sure if This is the premium reverse Doubling Error or not.
The A of STATES is weaker many times because it is across from Lincoln's shoulder. The depth of the die in this area tends to limit metal flow into STATES. On wheat cents, the same effect happens to the O of ONE.
I'm not sure how the "A's" of AMERICA are different in your images.

Yep, Pete got this one handled, a weak strike on the reverse is fairly common due to the lack of enough metal to form the reverse devices. The Mint 'fixed' this problem when they changed the obverse design to a low profile one.
