The Deutsche Mark gained a reputation for stability and Teutonic efficiency over its 50-year existence before it was replaced by the Euro. But even the German Bundesbank made mistakes... The first 2-Mark coin issued by the Federal Republic of Germany in 1951 had a virtually identical obverse to the 1-Mark, first issued in 1950, and was only slightly larger and heavier...  Not surprisingly, the coin caused chaos and I imagine many shopkeepers and streetcar conductors gave incorrect change when one of these was tendered in a busy shop or trolley car. The coin was promptly withdrawn and never issued again after 1951. Subsequent 2-Mark coins were larger and had the portrait of a famous German on the obverse (starting with Max Planck), making it impossible to confuse them with the 1-Mark. The 1951 coins are now quite scarce as their high face value meant that most were returned to the Mint for re-melting. This tempted a German Mint official to get hold of some of the original dies and re-strike a number of 1951-G 2-Mark coins for personal profit in the mid-1970s!
