1984 P Washington Quarter Neat Damage.

I figure this won't be a mint error, but for a long time I thought it was. My last coin was a bust I know this will be too.... But how was this damage created? Anyone seen anything like it? From the reverse it was obviously spun around real fast, chopping off the eagle's head. The obverse looks like they attached a bottle cap to a drill, in order to spin the coin. Very Neat to me... plus it's my birthyear.

It looks almost like someone hammered a bottle cap into the obverse, turned it slightly and then hit it again.

I know I've seen damage like this before, but I'm not sure of where. Time for some reverse image searching!
