Welcome to the forum,hope you enjoy it.As to your quarter it appears to me to be PMD.It kinda looks like it was sandwiched between 2 other quarters for a while creating that uneven toning.It is merely a guess on my part,but I am sure one of the many experts on here will help you out.

I really don't know (unless someone hit it with a hammer or something), but I'm wondering if the indentation and bulge (if that's what they are) line up with each other on the obverse and reverse? Seems like they wouldn't. Or are those just marks?

It may have been smacked between to pieces of wood with another coin on one or both sides. Not how the design is bent in slightly, but not like it would have been if put into a vise. If the coin is not bent, the it maybe a stain that we see on the coin. Nothing special as far as I can see.
