As far as what is going on, I believe it is all heavy Die Deterioration. Letters/devices moving outward and even touching or overlapping the rim is a good sign that the die used to strike that coin was quite worn and about due for retirement. I believe it even shows a crack running down from the point of Roosevelt's neck to the rim.

Also has a Broken Die Post (Die Chip) on the mint mark, do to the die breaking down.

 The post for the mintmark was broken on the die and so the result was a filled in letter D.
